- optional phone or WhatsApp number
- area or borough connection
- background, identity, or community connection responses
- wellbeing pulse responses

Why we collect it
We collect this information for listening-stage community research. It helps LSR understand what support may be needed, what safe spaces could look like, and how future men's circles, listening sessions, wellbeing spaces, or community insight work might be shaped.
This work is not clinical care, therapy, diagnosis, or a crisis service.
Our lawful basis
Where we process personal data, we rely on consent and/or legitimate interests for listening-stage community research, depending on the type of response and whether contact details are provided.
Some responses may include special category data, for example information about racial or ethnic background, identity, health, wellbeing, or lived experience. Where this applies, we rely on explicit consent given through the form before you continue.
You can choose not to answer any question you do not feel comfortable answering, and you can exit the form at any time without penalty.
How we use responses
We may use responses to:
- understand patterns and themes in community wellbeing
- shape future men's circles, listening sessions, or wellbeing spaces
- produce anonymous or public-safe summaries
- inform public-safe insight, funding, or partnership conversations
- improve LSR's community research and support design
We will not knowingly publish names or identifying personal details without explicit consent.
Who we may share information with
The Men's Wellbeing & Community Support Check-In is currently hosted through a third-party form platform, Tally.so. Tally's privacy policy may also apply to information submitted through that platform.
Responses may be exported to Google Sheets or similar tools for analysis by LSR.
We do not sell personal data. We do not share data for unrelated marketing purposes.
If anonymised summaries are shared publicly or with partners or funders, they will not knowingly include names or identifying personal details.
Retention
We will review form responses at the end of the listening phase. Contact details will only be kept for as long as needed to follow up about this work, future men's circles, listening sessions, wellbeing spaces, or related community activity.
You can ask us to delete your contact details at any time by emailing info@lsrmediahouse.org.uk.
Under-18s
The Men's Wellbeing & Community Support Check-In is primarily intended for adults engaged in community listening around men's wellbeing. If someone under 18 reaches the form, they can share anonymous views, but this version of the form is designed not to ask them for contact details or invite them to offer support.
